PBIS Parent Request for Behavior Support and Interventions
If you have any concerns about your child’s behavior, we encourage you to connect with their teacher and/or our school counselor. We’re here to support you and your child every step of the way!
Initial Contact: | · Teacher/Counselor will schedule a meeting with the parent to discuss their concerns about their child’s behavior. · Parents will provide specific examples and details of the problematic behavior, including when, where, and with whom it occurs. |

Data Collection: | · Parents will work with the school team to gather data on the child’s behavior through observation, formative assessment, continual assessment, and anecdotal records. · If necessary, interventions will be discussed and created to address the behavior concerns. |
Implementation and Monitoring: | · The teacher and/or counselor will monitor interventions. · Regular communication with parents will ensure they are informed about the progress of interventions and any necessary adjustments. |
Developing a Behavior Intervention Plan: | · If the behavior continues, escalates or other behaviors occur, the student may be moved into a Tier 3 Behavior Intervention Plan. |
